All well and good, so in preparation for forgetting about it I sent a test subscribe message from my 
Yahoo account to rnbwpnt-podc-subscribe <at> goldblatt.net and got this, and it landed in my regular mailbox 
and was not caught by ezmlm.

Patches for ezmlm-idx?

Hello,

I'm wondering what are the current patches to bring ezmlm-idx up to date 
and where to find them? I've downloaded the version of ezmlm-idx at 
http://www.ezmlm.org/ but have had problems getting moderated lists to work.

In a Google search, I saw a reference to patches to ezmlm-idx from a 
mailing list message from 2001. But, the patches are no longer at the 
referenced Website, but others are listed. That made me think that there 
may be updates to ezmlm-idx that I should apply in addition to the 
referenced, but no longer existing, ones.

So, what patches should I apply and where are they?

What is this about?  What should have been caught by ezmlm?  You seems
to have sent a message to a list's subscriber address.

You should have quoted the whole thread---most people cannot remember
2 week old messages.

I now see that this probably has nothing to do with filesystem: in the
database, you seem to have the correct subscriber list, and _my_
ezmlm-list shows also no duplicates.

What is your ezmlm version (ezmlm-make -v), and did yu apply any
patches besides the idx patch?

Is there anybody else here using OS X?

The version I'm using is:
ezmlm-make version: ezmlm-0.53+ezmlm-idx-0.40

And I didn't apply any other patches..

Ah hah!  Went searching again and found the solution to my problem.   
The problem was caused by the fact that MacOS X HFS+ is case  
insensitive, thus T is t, and t is also t... and so both are read.
