Minor/Major numbers are correct.

Yes true. So the problem is that the drive's not seen by the system for
whatever reason

No, not at all
And that's what we need to solve.

It's not a problem as the init script of mindi will later engage it as
well as cciss1.

Could you also send me that log file please. I'd like to see what it
says around the engage order.

Yep. The drive is not seen by the kernel at all.
Could you also provide the result of a dmesg just after a normal boot
of your system ?
