"mh" bundle split up into "mathtools" and "breqn" bundles

The well known "mh" bundle has been split into the

  mathtools bundle  maintained by Lars Madsen

and the

  breqn bundle maintained by Will Robertson.

Announcement of the mathtools bundle:

The mathtools bundle consists of the mathtools and empheq
packages (formerly a part of the mh bundle). Both packages

 The class now meets the formatting requirements as of the 2014
 Formatting Guide.  Among the highlights of the changes from the
 previous version due to changes by the Graduate School thesis
 office bureaucrats:

  - 'by' is now capitalized to 'By'

  - Degree and Field of study now on the same line

  - All TOC entries are now in bold so that they match the
    formatting of the headings themselves. (I'm not making this
    up!) To combat this I've added a [noboldtoc] option to turn
    it off. 

 It's so much better now.
